When you have the "Catalog Request" button on your Yahoo! Store nav-bar and allow your customers to request a newsletter or catalog, you can edit the request form through the Order Form page of the Store Manager.

The "Form Title" field allows you to change the name on the catalog request form. You can use the "Message" field to include special text or instructions.

Once customers have filled out the catalog request form, you can process the information through the "Requests" link on the Store Manager page.
