In order to separate the customer first and last name on the Order Form you need to select this option in the Order Form General Options.
To separate first and last name on the Order Form:
- From the Store Manager, select the "Order Form" link under the Order Settings column.
- Select "separate first and last name fields" from the "All forms use" drop-down menu of the General Order Form Options section
- Click [Done].
- Click the "Publish Order Settings" link to make these changes live on your store Order Form.
