The following are required sections that must be included on your Catalog Request form:

Sections: Catalog Request, Shipping Address

Fields: Name, Street Address 1, City, State, Zip Code, Country (all fields are located in the Shipping Address section)

You will not be able to hide or remove these sections and fields in Checkout Manager.
